dc.description.abstract | Los pacientes sometidos a trasplante renal presentan elevada morbimortalidad relacionada con sus patologías de base, procedimiento quirúrgico y terapia inmunosupresora. Estas situaciones requieren de parte de sus servicios tratantes intervenciones precisas y oportunas que garanticen que los esfuerzos y recursos utilizados tengan resultados óptimos. La valoración preparatoria busca mantener un paciente candidato en ideales condiciones para recibir al mejor injerto disponible, esto significa condiciones antropomórficas similares y mas difícil aun Condiciones inmunológicas similares. Este análisis se realiza teniendo en cuenta principalmente los complejos de histocompatibilidad mayor y Grupo sanguíneo intentando lograr la mejor compatibilidad posible. Sin embargo debido a el complejo polimorfismo de este sistema, las barreras geográficas para el traslado de órganos y las cortas ventanas de tiempo para realizar los trasplantes hacen que lograr una pareja de donante receptor ¨ideal¨ sea la excepción y no la norma. En nuestra institución se realiza durante el postoperatorio ademas del riguroso seguimiento clínico una valoración con ultrasonido del riñón trasplantado para identificar tempranamente complicaciones y con el método Doppler pulsado se estudia la morfología de las ondas obtenidas en los diferentes segmentos arteriales intrarenales, para de esta forma obtener una aproximación a diferenciar un estado fisiológico de uno patológico. El análisis de la morfología de ondas e índices de resistencia pueden aportar indicios o soportar sospechas acerca de la disfunción temprana del injerto y/o en el seguimiento del mismo. El estudio histopatológico de una muestra de tejido obtenido mediante una punción guiada del riñón trasplantado es en pacientes seleccionados la forma definitiva de hacer el diagnóstico sin embargo al ser un estudio invasivo y no exento de complicaciones se desean plantear alternativas que permitan diferenciar de forma oportuna a estos pacientes y permitir realizar intervenciones o modificar la terapéutica. | spa |

dc.description.abstractenglish | Patients who undergo renal transplantation become a group with elevated morbidity and mortality related to pathology, surgical procedure and inmunosuppressant treatment. All of this clinical situations demand working groups which provide precise and opportune interventions to warrant that the efforts and resources turn into optimal results. The preparatory assessment seeks to maintain a candidate patient in ideal conditions to receive the best graft available, this means similar anthropomorphic conditions and even more difficult Similar immunological conditions. This analysis is carried out taking into account mainly the major histocompatibility COMPLEXES and blood group trying to achieve the best possible compatibility. However, due to the complex polymorphism of this system, the geographical barriers for the transfer of organs and the short windows of time to perform transplants make that achieving an ¨ideal¨ donor-partner couple is the exception and not the norm. In our institution, during the postoperative period, in addition to the rigorous clinical follow-up, an ultrasound evaluation of the transplanted kidney is performed to identify early complications. With the pulsed Doppler method, the morphology of the waves obtained in the different intrarenal arterial segments is studied, in order to obtain an approach to differentiate a physiological state from a pathological one. The analysis of wave morphology and resistance indices can provide clues or support suspicions about early graft dysfunction and / or follow-up. The histopathological study of a tissue sample obtained through a guided puncture of the transplanted kidney is the definitive way of making the diagnosis in selected patients.
