dc.description.abstractEl glaucoma es una causa importante de ceguera irreversible alrededor del mundo. En el 2010, se reportó 6.5% de ceguera debido al glaucoma. Sin embargo, la ceguera puede ser prevenible realizando un diagnóstico temprano y estableciendo un tratamiento oportuno. Esta neuropatía óptica deriva de varios factores de riesgo que incluyen el incremento de la presión intraocular. En la actualidad, el único enfoque que ha probado ser eficiente en preservar la función visual ha sido la disminución de los niveles de presión intraocular, es por esta razón que, es de suma importancia llevar a cabo un control adecuado de la misma. Objetivo: Evaluar los resultados postoperatorios de implante de válvula de Ahmed en glaucoma de difícil manejo. Material y métodos: Análisis secundario de un estudio descriptivo de tipo observacional longitudinal retrospectivo. Se incluyeron pacientes con glaucoma de difícil manejo, que fueron intervenidos de implante de dispositivo de drenaje en la Fundación Oftalmológica de Santander — FOSCAL o FOSUNAB entre julio del 2013 a junio del 2017. Resultados: La mediana de la edad fue 68.5 años. El 59% fueron hombres y el 41% mujeres. Antecedentes oftalmológicos se reportó ojo único (17%), retinopatía diabética (14%) y la oclusión venosa de la retina (14%) dentro de los más frecuentes. El 54% fue por glaucoma de ángulo abierto de difícil manejo, seguido del neovascular con un 23%. El 43.55% de los ojos tenían antecedente de trabeculectomía y el 77.42% cirugía de catarata. La mediana de la PIO preoperatoria fue de 28 mmHg, a los 6 meses de 16 mmHg y al último control de 15 mmHg (p= < 0.001). 4 hipotensores antes del procedimiento y al último control 3 (p=0.001). Se evidencio reducción de más del 50% de la proporción de éxito absoluta alrededor de la tercera semana. Las complicaciones más frecuentes fueron el edema corneal, el hifema y la hipotonía. Conclusiones: El implante de la válvula de Ahmed en el glaucoma de difícil manejo es una alternativa quirúrgica de buena elección, evidenciándose la reducción de la presión intraocular y número de hipotensores durante el seguimiento sin mayores tasas de complicaciones tardías.spa

However, blindness can be preventable by making an early diagnosis and presenting a timely treatment. This optic neuropathy derives from several risk factors that include increased intraocular pressure. At present, the only approach that has proven to be efficient in preserving visual function has been the decrease in intraocular pressure levels, for this reason it is extremely important to carry out adequate control of it. Objective: To evaluate the postoperative results of Ahmed valve implantation in difficult-to-manage glaucoma. Material and methods: Secondary analysis of a retrospective longitudinal observational descriptive study. Patients with difficult-to-manage glaucoma were included, who underwent drainage device implantation at the Fundación Oftalmológica de Santander — FOSCAL or FOSUNAB between July 2013 and June 2017. Results: The median age was 68.5 years. 59% were men and 41% women. Ophthalmological history reported single eye (17%), diabetic retinopathy (14%) and retinal vein occlusion (14%) among the most frequent. 54% were due to difficult-to-manage open-angle glaucoma, followed by neovascular glaucoma with 23%. 43.55% of the eyes had a history of trabeculectomy and 77.42% cataract surgery. The median preoperative IOP was 28 mmHg, 16 mmHg at 6 months, and 15 mmHg at the last control (p= < 0.001). 4 hypotensive before the procedure and 3 at the last control (p=0.001). A reduction of more than 50% of the absolute proportion was evidenced around the third week. The most frequent complications were corneal edema, hyphema, and hypotonia. Conclusions: The implantation of the Ahmed valve in difficult-to-manage glaucoma is a surgical alternative of good choice, evidencing the reduction of intraocular pressure and the number of hypotensives during follow-up without higher rates of late complications.eng
