dc.description.abstract | En los últimos años se han realizado múltiples estudios que posicionan al DMEK como la técnica más segura de trasplante corneal, ya que presenta un porcentaje de rechazo inferior al 1%, números que contrastan con los porcentajes de rechazo del DSEK cercanos al 10% y casi el 20% para las KP. Se tienen también como datos positivos comparativos del DMEK con DSEK y otras técnicas: Una disminución de la pérdida de las células endoteliales con el pasar del tiempo, una mejoría más rápida de la agudeza visual, y un mejor pronóstico visual. Teniendo esto como punto de partida, Melles y otros autores a nivel mundial, han realizado estudios con el fin de determinar si la experticia del cirujano en la técnica quirúrgica puede afectar positiva o negativamente los resultados hasta ahora obtenidos. Melles et al concluyen que no es la experticia que el cirujano tenga en la técnica quirúrgica la que determina el resultado exitoso del DMEK, pero si la obtención del injerto a trasplantar. Sin embargo, aún existe mucha controversia sobre los determinantes del éxito del DMEK, especialmente aquellos relacionados con la técnica quirúrgica. Es por eso que este estudio se plantea como objetivo realizar un análisis retrospectivo basado en un análisis secundario de una base de datos existente de aquellos pacientes a los cuales se les ha realizado DMEK en el Centro Oftalmológico Virgilio Galvis Ramirez y han cumplido con 12 meses de seguimiento postoperatorio, y evaluar la curva de aprendizaje de dicho procedimiento. | spa |

dc.description.abstractenglish | In recent years, multiple studies have been carried out that position DMEK as the safest technique for corneal transplantation, since it presents a rejection percentage of less than 1%, numbers that contrast with the rejection percentages of DSEK close to 10% and almost 20% for KP. There are also comparative positive data of DMEK with DSEK and other techniques: A decrease in the loss of endothelial cells over time, a faster improvement in visual acuity, and a better visual prognosis. Taking this as a starting point, Melles and other authors worldwide have carried out studies in order to determine whether the surgeon's expertise in the surgical technique can positively or negatively affect the results obtained so far. Melles et al conclude that it is not the expertise of the surgeon in the surgical technique that determines the successful outcome of DMEK, but rather obtaining the graft to be transplanted. However, there is still much controversy about the determinants of the success of DMEK, especially those related to the surgical technique. That is why this study aims to carry out a retrospective analysis based on a secondary analysis of an existing database of those patients who have undergone DMEK at the Virgilio Galvis Ramirez Ophthalmology Center and have completed 12 months of postoperative follow-up, and evaluate the learning curve of said procedure. | eng |
