dc.description.abstract | Objetivo: Estudiar los resultados visuales, la pérdida de células endoteliales y las complicaciones en pacientes a quienes se les realizó un trasplante de córnea con la técnica Dsaek. Materiales y métodos: En este estudio observacional prospectivo se incluyeron todos los pacientes a quienes se les practicó Dsaek desde que esta técnica fue implementada en en la FUNDACION OFTALMOLOGICA DE SANTANDER – CLINICA CARLOS ARDILA LULLE (FOSCAL), en abril de 2009. Se evaluaron los resultados visuales, la transparencia y el recuento endotelial de los botones donantes en el postoperatorio. Resultados: Se realizó un estudio de tipo observacional de corte transversal, en donde se tomaron 35 ojos de 35 pacientes. Promedio de edad 67.5 ± 10.7 años. 16 ojos de pacientes sexo femenino (45.7 %) y 19 ojos de pacientes con sexo masculino (54.3 %). El tiempo de seguimiento promedio fue de 12.2 +/- 6.7 (rango de 1.3 a 28.7) meses. El promedio de agudeza visual con corrección (AVcc) prequirúrgica (n=35) fue 1,51 +/- 0,72 LogMAR (aproximadamente 20/647 en notación de Snellen), y el promedio en el control realizado después de un año de cirugía (n=24) fue 0,94 +/- 0,78 LogMAR (aproximadamente 20/174). En este último control el 62.5 % (n=15) de los casos mejoraron sus AVcc con respecto a la prequirúrgica; el 25% (n=6) no tuvieron variación con respecto a su AVcc inicial y el 12.5% (n=3) terminaron con una visión menor que la preoperatoria. En este control el recuento endotelial promedio de las córneas trasplantadas fue 1718 +/- 6.51 (rango de 807 – 3045) células/mm2. El 17.1% (n= 6) de los casos presentaron falla del trasplante (2 por rechazo y 4 por falla primaria). El defecto astigmático promedio en el control realizado al menos un año después del procedimiento fue -1.74 D. Desviacion Estandar +/- 1.78 Min 0, Max 7. Dos ( 2 ) ojos requirieron reintervenciones. Conclusiones: El DSAEK es una alternativa viable para realizar el trasplante de tejido corneal en casos de falla endotelial, pero está asociado a riesgos inherentes que pueden limitar la recuperación visual. | spa |

Materials and methods: In this prospective observational study included all patients who underwent DSAEK since this technique was implemented in the EYE FOUNDATION OF SANTANDER - CARLOS ARDILA LULLE CLINIC (FOSCAL) in April 2009. We evaluated the visual results, transparency and counting the buttons donor endothelial postoperatively. Results: An observational study of cross section, where it took 35 eyes of 35 patients.Mean age 67.5 ± 10.7 years. 16 eyes of female patients (45.7%) and 19 eyes of male patients (54.3%). The average follow-up time was 12.2 + / - 6.7 (range 1.3 to 28.7) months. The mean corrected visual acuity (ETDRS), preoperative (n = 35) was 1.51 + / - 0.72 LogMAR (approximately 20/647 in Snellen notation), and the average check carried out after a year of surgery (n = 24) was 0.94 + / - 0.78 LogMAR (approximately 20/174). In this last control 62.5% (n = 15) of its AVcc cases improved compared to the preoperative, 25% (n = 6) did not change from its initial AVcc and 12.5% (n = 3) ended with less than the preoperative vision. In this control the mean endothelial count transplanted corneas was 1718 + / - 6.51 (range of 807 - 3045) cells/mm2. 17.1% (n = 6) of cases had the transplant fails (2 rejection and 4 primary failure). The average astigmatic defect in the control carried out at least one year after the procedure was -1.74 D. Standard deviation + / - 1.78 Min 0, Max 7. Two (2) eyes required reoperation. Conclusions: The DSAEK is a viable alternative for corneal tissue transplantation in cases of endothelial failure, but is associated with inherent risks that may limit visual recovery. | eng |
