dc.description.abstract | Antecedentes: El dengue constituye un problema prioritario de salud pública, por su severidad ampliamente descrita y estudiada, y por la existencia de otros factores que incrementan el impacto de la enfermedad como la mortalidad y la discapacidad. Todo esto, define al dengue como una condición con implicaciones en sus fases aguda y convaleciente. Además de la clínica, aunque se han identificado marcadores biológicos de severidad, no se ha explorado el papel que podrían tener estos en la predicción de complicaciones durante la convalecencia. Objetivo: Determinar la asociación entre la severidad clínica, la concentración de un marcador inflamatorio y la antigenemia Ns1, con la afectación de la calidad de vida relacionada con la salud (CVRS) y la fatiga, en la fase convaleciente de la infección por virus del dengue. Metodología: Partiendo de un estudio de pronóstico ensamblado durante el año 2019 (epidémico para dengue en Colombia), se evaluó la relación entre las características clínicas de la fase aguda, la concentración de proteína C reactiva (PCR, mg/dl) y la antigenemia Ns1 (unidades Panbio), con la CVRS (Kidscreen-52 [niños]; SF-36 [adultos]) y la incidencia de fatiga (ME-CFS; Chalder [CFQ-11]) determinados al 1 y 6 mes de seguimiento. Estas relaciones fueron evaluadas mediante técnicas de regresión, tanto lineal como logística, ajustando por confusores (edad, sexo, infección secundaria y comorbilidades). Resultados: En una muestra de 253 participantes con dengue, el 78% fueron menores de edad (2 a 17 años). Los cuestionarios de fatiga y calidad de vida fueron aplicados a 127 (50.2%) participantes entre niños y adultos. En niños, la CVRS en términos de las dimensiones Estado de ánimo y Entorno escolar, tienen una relación directamente proporcional con los niveles de inflamación en la fase aguda (1.53 puntos, <0.001, y 0.38 puntos, p= 0.022; respectivamente), siendo el estado de ánimo una dimensión en la que se observaron bajos niveles de calidad de vida a lo largo del seguimiento en convalecencia. Los altos puntajes en entorno escolar, se asociaron con bajos niveles de Ns1 en la fase aguda (0.21 puntos menos, p= 0.013). En cuanto a los adultos, ni la PCR ni la Ns1 parecen asociarse con cambios en los puntajes de evaluación de la CVRS ni en su componente físico ni mental. Por otro lado, en el análisis de fatiga se encontró que el 33.9% (n= 43, n= 22 menores y n=21 adultos) de la muestra tuvo un puntaje compatible con esta condición a los 6 meses de la fase aguda. En los niños/adolescentes se observó que es explicada por el aumento en los niveles de antigenemia y el aumento de la edad (OR: 1.06, IC95% 1.01-1.12, y 1.33 IC95% 1.09-1.62; respectivamente); mientras que en los adultos no se encontró que la fatiga en fase convaleciente tardía esté explicada por los niveles de Ns1. En ninguno de los dos grupos de edad se encontró contribución de los niveles de inflación con el desarrollo de fatiga post dengue. Conclusiones: En los niños/adolescentes de la muestra, los niveles de inflamación en la fase aguda tienen efecto adicional a la severidad clínica en la calidad de vida de la fase convaleciente en términos del estado de ánimo, mientras que el aumento en los niveles de antigenemia Ns1 y de la edad, se asocian independientemente de la severidad clínica, con el desarrollo de fatiga en fase convaleciente tardía. Este estudio aporta a la caracterización de la fase convaleciente del dengue en una muestra en la que prevalecen los menores de edad, lo que permite abordar una arista importante de la carga subestimada del dengue en población infantil y juvenil durante una epidemia, un área de investigación poco atendida. | spa |

dc.description.abstractenglish | Background: Dengue constitutes a priority public health problem, due to its widely described and studied severity, and the existence of other factors that increase the impact of the disease such as mortality and disability. All this defines dengue as a condition with implications in its acute and convalescent phases. Although biological markers of severity have been identified, the role that these could have in the prediction of complications during convalescence has not been explored. Objective: To determine the association between clinical severity, the concentration of an inflammatory marker and Ns1 antigenemia with health-related quality of life (HRQoL) and fatigue in the convalescent phase of dengue virus infection. Methodology: Based on a prognostic study conducted during 2019 (epidemic year for dengue in Colombia), the relationship between clinical characteristics of the acute phase, C-reactive protein concentration (CRP, mg/dl) and Ns1 antigenemia (Panbio units), with HRQoL (Kidscreen-52 [children]; SF-36 [adults]) and fatigue incidence (ME-CFS; Chalder [CFQ-11]) determined at 1 and 6 months of follow-up was evaluated. These relationships were evaluated using both linear and logistic regression techniques, adjusting for confounders (age, sex, secondary infection and comorbidities). Results: In a sample of 253 participants with dengue fever, 78% were under age (2 to 17 years). Fatigue and quality of life questionnaires were applied to 127 (50.2%) participants among children and adults. In children, HRQOL in terms of the dimensions mood and school environment had a direct proportional relationship with the levels of inflammation in the acute phase (1.53 points, <0.001, and 0.38 points, p= 0.022; respectively), with mood being a dimension in which low levels of quality of life were observed throughout the convalescence period. High scores in school environment were associated with low levels of Ns1 in the acute phase (0.21 points less, p= 0.013). As for adults, neither CRP nor Ns1 seemed to be associated with changes in HRQoL scores in either the physical or mental component. On the other hand, in the fatigue analysis it was found that 33.9% (n= 43, n= 22 minors and n=21 adults) of the sample had a score compatible with this condition 6 months after the acute phase. In children/adolescents it was observed to be explained by increasing antigenemia levels and increasing age (OR: 1.06, 95%CI 1.01-1.12, and 1.33 95%CI 1.09-1.62; respectively); while in adults fatigue in the late convalescent phase was not found to be explained by Ns1 levels. In neither age group was a contribution of inflation levels found with the development of post dengue fatigue. Conclusions: In the children/adolescents in the sample, levels of inflammation in the acute phase have an effect in addition to clinical severity on convalescent phase quality of life in terms of mood, while increased levels of Ns1 antigenemia and age, independently of clinical severity, are associated with the development of fatigue in the late convalescent phase. This study contributes to the characterization of the convalescent phase of dengue in a sample in which minors are prevalent, which allows us to address an important aspect of the underestimated burden of dengue in children and young people during an epidemic, an area of research that has received little attention. | spa |
