dc.description.abstract | Introducción: La enfermedad por reflujo gastroesofágico (ERGE) es una enfermedad con una alta prevalencia que afecta la calidad de vida y se considera una patología silente debido a que la mayoría de los pacientes no buscan atención médica y se automedican generando un retraso en el diagnóstico y tratamiento oportuno. El planteamiento del problema de investigación de este estudio se debe a que durante la realización de estudios baritados en pacientes sanos se observó una asociación frecuente entre la ERGE y los residuos faríngeos. Actualmente no hay estudios disponibles que demuestren una asociación positiva entre estos hallazgos. Los residuos faríngeos son un predictor importante de penetración y broncoaspiración, por lo tanto, si logramos establecer una asociación positiva de la ERGE con los residuos, también podríamos establecer que existe un mayor riesgo de presentar penetración y aspiración en los pacientes que tienen ERGE. Objetivo: Evaluar la asociación entre los residuos faríngeos, el diagnóstico de ERGE y los factores asociados mediante la cinedeglución. Resultados: Se incluyeron todos los pacientes registrados en una base de datos con diagnóstico de ERGE que hayan acudido al servicio de radiología FOSCAL para realización de estudio baritado y se excluyeron pacientes con antecedentes quirúrgicos a nivel de faringe, esófago o UGE, pacientes con trastornos de la deglución y pacientes menores de edad. Se analizó una base de datos de 137 pacientes sanos, el 43% mujeres, el 56% hombres, la edad promedio fue de 57,6 años (21-85 años), la prevalencia estimada de ERGE en nuestra población fue del 17% de los cuales el 83,9% de los pacientes tuvieron residuos faríngeos, el grado mas frecuente de reflujo fue el 2 y el 3, las comorbilidades asociadas con mayor frecuencia fueron la hipertensión arterial, el EPOC, la enfermedad tiroidea, la diabetes mellitus y la obesidad. Los síntomas principales fueron la tos, la sensación de atoro, ronquido, disfonía y pirosis. Presentaron penetración el 10,9% y aspiración e 2,1%. Los grados menores de reflujo predominaron en los extremos de la edad y los grados altos entre los 45-65 años, estos últimos con mayor cantidad de residuos. El 100% de los pacientes con residuos tenía obesidad. El 100% de los pacientes que tenían penetración y aspiración tuvieron residuos faríngeos y un grado de reflujo mayor de 3. Conclusiones: La prevalencia del reflujo gastroesofágico en nuestra población se encuentra dentro de un rango similar a lo reportado en el marco global y en América Latina. La prevalencia de los residuos faríngeos en pacientes con ERGE sin trastornos neurológicos ni de la deglución fue del 83,9%, lo cual nunca había sido descrito en la literatura y su importancia clínica radica en la asociación que tiene con la broncoaspiración y la penetración. Se demostró un gradiente biológico entre la severidad del RGE y los residuos faríngeos. Los pacientes que tienen residuos faríngeos presentan más penetración y broncoaspiración que los que no los tienen. No solo logramos establecer una relación indirecta entre la ERGE y la penetración/aspiración producido a través del acumulo de residuos, que era la hipótesis inicial, sino que también demostramos que hay una tendencia a que entre más alto sea el reflujo mayor probabilidad de penetración laríngea y aspiración. Este se convierte en el primer estudio que aporta datos sobre ERGE, residuos faríngeos y sus factores asociados en una población colombiana y que es consistente con los estudios en otros países. | spa |

dc.description.abstractenglish | Introduction: Gastroesophageal reflux disease (GERD) is a disease with a high prevalence that affects quality of life and is considered a silent pathology because most patients do not search medical attention and self-medicate, generating a delay in diagnosis and timely treatment. The research problem in this study was that during barium studies in healthy patients, a frequent association between GERD and pharyngeal residues was observed. Currently there are no studies available that demonstrate a positive association between these findings. Pharyngeal residues are an important predictor of penetration and aspiration, therefore, if we can establish a positive association of GERD with residues, we could also establish that there is a greater risk of presenting penetration and aspiration in patients who have GERD. Objective: To evaluate the association between pharyngeal residues, the diagnosis of GERD and the associated factors by barium studies. Results: All patients registered in a database with a diagnosis of GERD who had attended the FOSCAL radiology service for a barium study were included and. Patients with a surgical history at the pharynx, esophagus or GEJ and patients with swallowing disorders were excluded. A database of 137 healthy patients was analyzed, 43% women, 56% men, the average age was 57.6 years (21-85 years), the estimated prevalence of GERD in our population was 17 % of which 83.9% of the patients had pharyngeal residues, the most frequent degree of reflux was 2 and 3, the most frequently associated comorbidities were arterial hypertension, COPD, thyroid disease, diabetes mellitus and obesity. The main symptoms were coughing, choking sensation, snoring, dysphonia and heartburn. 10.9% had penetration and 2.1% aspiration. The lower degrees of reflux predominated at the extremes of age and the high degrees between 45-65 years, this one with the highest amount of residues. 100% of the patients with residuals were obese. 100% of the patients who had penetration and aspiration had pharyngeal residues and a degree of reflux greater than 3. Conclusions: The prevalence of gastroesophageal reflux in our population is similar to reported in the global framework. The prevalence of pharyngeal residues in patients with GERD without neurological or swallowing disorders was 83.9%, which had never been described in the literature and its clinical importance is due to the association that has with bronchial aspiration and penetration. A biological gradient was demonstrated between the severity of the GER and the pharyngeal residues. Patients who have pharyngeal residues have more penetration and aspiration than those who don’t. The initial hypothesis was established because we found an indirect relationship between GERD and the penetration / aspiration produced through the accumulation of residues, but we also demonstrated that there is a relation between the reflux grade and the probability of laryngeal penetration and aspiration. This becomes the first study that provides data on GERD, pharyngeal residue and their associated factors in a Colombian population and that is consistent with studies in other countries. | eng |
