dc.description.abstractLos ensayos clínicos aleatorizados (ECA) que evalúan la terapia tripanocida (TT) en sujetos crónicamente infectados por Trypanosoma cruzi no registran desenlaces clínicos. Los estudios observacionales (EOBS) constituirían una alternativa para informar las decisiones clínicas. Métodos: Identificamos sistemáticamente EOBS que registraran la mortalidad de cohortes tratadas y no tratadas con TT (TT+ y TT- respectivamente), consultando MEDLINE, LILACS, referencias de publicaciones, expertos y la industria farmacéutica. Se evaluó la calidad de los EOBS incluidos y se extrajeron sus datos en desenlaces clínicos. El tamaño del efecto se calculó usando el abordaje de modelos aleatorios. Resultados: Cinco EOBS (n = 2096, seguimiento de 5-14 años) registraron mortalidad en las cohortes(19/827 en TT+ versus 62/1269 en TT-, OR = 0.55, IC95% 0.17, 1.80) con resultados heterogéneos entresi (χ2 = 14.1, p = 0.007). Los 3 estudios con mejor evaluación, o los 4 conducidos en el hemisferio sur, contenían resultados homogéneos (p = 0.5 y p = 0.2, respectivamente), pero discordantes (OR = 1.49,IC95% 0.69, 3.24 y OR = 0.35, IC95% 0.13, 0.97, respectivamente). Dos EOBS registraron la aparición de falla cardiaca y alteraciones electrocardiográficas (8 y 11 eventos respectivamente, OR < 1, p > 0.1 en ambos casos). Ningún EOBS manejó las variables de confusión para estimar el efecto de la TT. Conclusiones: El agregado de EOBS muestra una reducción no significativa de los desenlaces clínicos en sujetos TT+. Los resultados heterogéneos en mortalidad, el escaso número de eventos clínicos registrados y los problemas metodológicos de los EOBS no sustentarían la recomendación de TT, pero estimulan el diseño de ECA registrando desenlaces clínicos en esta población. dc.description.abstractenglishRandomized clinical trials (RCTs) evaluating trypanocidal therapy (TT) in chronically infected subjects with Trypanosoma cruzi do not record clinical outcomes. Observational studies (EOBS) would constitute an alternative to inform clinical decisions. Methods: We systematically identified EOBS that recorded the mortality of cohorts treated and not treated with TT (TT + and TT- respectively), consulting MEDLINE, LILACS, references of publications, experts and the pharmaceutical industry. The included EOBS were assessed for quality and their data on clinical outcomes were extracted. The effect size was calculated using the random model approach. Results: Five EOBS (n = 2096, 5-14 years follow-up) recorded mortality in the cohorts (19/827 in TT + versus 62/1269 in TT-, OR = 0.55, 95% CI 0.17, 1.80) with heterogeneous results between ( χ2 = 14.1, p = 0.007). The 3 studies with the best evaluation, or the 4 conducted in the southern hemisphere, contained homogeneous results (p = 0.5 and p = 0.2, respectively), but discordant (OR = 1.49, 95% CI 0.69, 3.24 and OR = 0.35, 95% CI 0.13 , 0.97, respectively). Two EOBS recorded the occurrence of heart failure and electrocardiographic abnormalities (8 and 11 events respectively, OR <1, p> 0.1 in both cases). Neither EOBS handled confounding variables to estimate the effect of TT. Conclusions: The addition of EOBS shows a non-significant reduction in clinical outcomes in TT + subjects. The heterogeneous results in mortality, the low number of registered clinical events and the methodological problems of the EOBS would not support the recommendation of TT, but stimulate the design of RCTs recording clinical outcomes in this population.
