dc.description.abstract | Objetivo: Describir el manejo médico expectante ambulatorio en pacientes con ruptura prematura de membranas (RPM) entre las semanas 24 y 34 de edad gestacional. Métodos: Se realizó un estudio observacional de las pacientes con Ruptura Prematura de Membranas con manejo médico expectante ambulatorio. Resulta-dos: En el presente estudio fueron incluidas 14 pacientes con edades entre 20 y 39 años con RPM entre las 25 y 33 semanas con una duración promedio del manejo expectante de 38 días. Las indica-ciones para terminar la gestación fueron: la presencia de actividad uterina espontánea en siete (50%) casos, infección intraamniótica en un (7.14%) caso, colestasis intrahepática un (7.14%) caso y embarazo a término en cuatro (28.5%) casos. Una (7.14%) paciente presentó cierre espontáneo de la ruptura de membranas y parto normal en semana 39. La vía del parto fue definida por indicación obstétrica de tal forma que en nueve pacientes el parto fue normal y cesárea en cinco pacientes, de las cuales una fue por inducción fallida y cuatro por presentación podálica. Conclusiones: El manejo médico expectante es el tratamiento indicado para las pacientes con RPM sin signos clínicos de infección materna ni fetal con edades gestacionales entre 24 y 34 semanas. El manejo médico expectante ambulatorio es una opción adecuada para pacientes selecciona-das, sin embargo es necesario realizar más trabajos aleatorios controlados que permitan establecer que esta es una alternativa terapéutica al manejo médico expectante intrahospitalario. dc.description.abstractenglish | Expecting ambulatory medical treatment, among patients having premature rupture of membranes. An experience at the Fetal-Maternal Unit of The Reina Sofía Clinic. Objective: To describe the expecting ambulatory medical treatment in patients with Premature Rupture of Membranes (PRM), between 24 and 34 weeks of gestational age. Materials and methodology: An observational study was made in patients with PRM and expecting ambulatory medical treatment. Outcome: In this study, 14 patients between 20 to 39 years of age with PRM between 25 to 33 weeks of gestational age were included with a time period of 38 days of expecting treat-ment. The indications to terminate gestation were: presence of spontaneous uterine activity (7 patients, 50%), chorio-amnionitis (1 patient, 7.14%), intrahepatic cholestasis (1 patient, 7.14%), and full term delivery (4 patients, 28.5%). One of the patients presented a spontaneous closing of her membrane rupture and had a normal delivery on her 39th week of pregnancy. The delivery was defined by obstetric indication so that 9 patients had normal delivery and 5 patients had their deliveries by cesarean section, one of which was because of failed induction, and 4 of them due to a podalic presentation. Conclusions: The expecting ambulatory medical treat-ment is the one chosen for patients with PRM without clinical signs of maternal or fetal infection, and gestational ages between 24 to 34 weeks. Although the expecting ambulatory medical treatment is one of the correct choices, it is necessary that more controlled studies be conducted to establish whether or not it is a therapeutic of choice for the expecting hospital medical treatment | eng |
