dc.description.abstract | La Secretaría de Salud y del Ambiente de Bucaramanga viene desarrollando, como parte del proyecto Carmen, el Programa de Detección temprana de cáncer de mama, con el propósito de mejorar las condiciones de acceso de la población pobre no asegurada de los 20 barrios del área demostrativa. Se presentan los resultados del trabajo realizado en el 2005. Metodología: Estudio retrospectivo descriptivo de la prevalencia de hallazgos clínicos, mamográfica cos y ecográfica cos según la categorización Birads. El tamizaje incluyó interrogatorio, examen físico e imágenes en mujeres mayores de 40 años sin seguridad social, residentes en nueve barrios del área demostrativa del programa Carmen. Resultados: Se realizaron 1.054 procedimientos en 527 mujeres. El 53% de la población tuvo exámenes normales, mientras que el 40% tenían algún tipo de hallazgo benigno (cal-cifi caciones, lesiones nodulares sólidas o quistes). Las patologías benignas que se diagnosticaron con biopsia fueron fi broadeno-mas, cambios fi broquísticos, cambio fi broadenomatoide, mastitis crónica y adenoma tubular. Hubo lesiones diagnosticadas en la mamografía que no se observaron en la ecografía y viceversa, pero al combinar los dos métodos diagnósticos mejoraba la caracterización de las imágenes, la exactitud diagnóstica y los hallazgos. Conclusiones: el examen físico por sí solo, no es confiable para el tamizaje. La utilización de la ecografía en forma simultánea con la mamografía mejora la detección de lesiones mamarias y caracterización de las mismas. [Jaramillo N, Mantilla JC, Rey JJ. dc.description.abstractenglish | The Ministry of Health and Environment of Bucaramanga has been developing, as part of the Carmen project, the Breast Cancer Early Detection Program, with the purpose of improving access conditions for the uninsured poor population of the 20 neighborhoods of the demonstration area . The results of the work carried out in 2005 are presented. Methodology: Descriptive retrospective study of the prevalence of clinical findings, cos mammography and cos ultrasound according to the Birads categorization. The screening included questioning, physical examination and images in women over 40 years of age without social security, residing in nine neighborhoods of the Carmen program demonstration area. Results: 1,054 procedures were performed in 527 women. 53% of the population had normal examinations, while 40% had some type of benign finding (cal-ciptions, solid nodular lesions or cysts). The benign pathologies that were diagnosed with biopsy were fibroadenesmas, fibrocystic changes, fibroadenomatoid change, chronic mastitis and tubular adenoma. There were lesions diagnosed on mammography that were not seen on ultrasound and vice versa, but combining the two diagnostic methods improved image characterization, diagnostic accuracy, and findings. Conclusions: physical examination alone is not reliable for screening. The use of ultrasound simultaneously with mammography improves the detection of breast lesions and their characterization.
