dc.description.abstractEvaluar los resultados del implante secundario del lente intraocular (LIO) en cámara posterior en pacientes áfacos en menores de 18 años. Metodología: Revisión retrospectiva de pacientes atendidos entre diciembre/1998 y junio/2006 en cuanto a etiología de la catarata, diagnóstico preoperatorio, variables quirúrgicas complicaciones intraoperatorias y postoperatorias, y necesidad de reintervenciones. se midió agudeza visual no corregida (AVSC) y mejor corregida (AVCC) antes y después del implante secundario. Se analizó causa de no mejoría o empeoramiento visual luego de cirugía y error predictivo en biometría. Resultados: Se identificaron 74 ojos de 50 pacientes, 59 ojos con catarata congénita y 15 adquirida (14 por trauma); la edad media de lensectomía por catarata con génita fue 7 meses y por adquirida 7 años, para el implante secundario de 6 años en el primer grupo y 11 años en el segundo. La complicación postoperatoria más común fue opacificación del eje visual, que requirió reintervención quirúrgica o capsulotomía láser. La AVCC mejoró luego del implante secundario en 73% de los ojos y se mantuvo igual 17.5%. El seguimiento promedio fue 21 meses. Al último control, 44.8% de los ojos tuvieron AVCC mejor de 20/40 y 77.4% mejor de 20/100. Conclusiones: El implante secundario de LIO en cámara posterior es seguro y efectivo en el manejo de afaquia pediátrica. dc.description.abstractenglishTo evaluate the results of secondary intraocular lens (IOL) implantation in the posterior chamber in aphacic patients under 18 years of age. Methodology: Retrospective review of patients seen between December/1998 and June/2006 regarding cataract etiology, preoperative diagnosis, surgical variables, intraoperative and postoperative complications, and need for reinterventions. uncorrected (BCVA) and best corrected (BCVA) visual acuity were measured before and after secondary implantation. The cause of non-improvement or visual worsening after surgery and predictive error in biometrics were analyzed. Results: 74 eyes of 50 patients were identified, 59 eyes with congenital cataract and 15 acquired (14 due to trauma); the average age of lensectomy for cataract with congenital was 7 months and for acquired 7 years, for the secondary implantation of 6 years in the first group and 11 years in the second. The most common postoperative complication was opacification of the visual axis, which required surgical reintervention or laser capsulotomy. CCVA improved after secondary implantation in 73% of eyes and remained the same in 17.5%. The average follow-up was 21 months. At the last control, 44.8% of the eyes had BCVA better than 20/40 and 77.4% better than 20/100. Conclusions: Secondary IOL implantation in the posterior chamber is safe and effective in the management of pediatric aphakia.
